Levothyroxine Timing: The Coffee Rule + Empty Stomach Guide

Published
March 11, 2026
Key Takeaways
  • Take levothyroxine on an empty stomach 30-60 minutes before eating for proper absorption
  • Coffee reduces absorption by 29-36%, so wait at least 60 minutes or ask your doctor about bedtime dosing
  • Keep calcium, iron, and antacids at least 4 hours apart from levothyroxine. A calcium pill with breakfast is too close to a dose taken before breakfast
  • Bedtime dosing is a legitimate alternative if you finish dinner 3+ hours before bed
  • Pick one routine and keep it: the ATA recommends taking it consistently, either 60 minutes before breakfast or at bedtime

Take levothyroxine on an empty stomach, 30 to 60 minutes before eating. The FDA prescribing information says to take it "on an empty stomach, one-half to one hour before breakfast." Unlike atorvastatin or Lexapro where timing is mostly about preference, levothyroxine has real absorption rules. Get the timing wrong and you absorb less of each dose.

Why Levothyroxine Timing Actually Matters

With most medications in this series, we’ve said to take it whenever works for you. Levothyroxine is the exception.

The Synthroid label says your body absorbs 40% to 80% of each dose, and fasting raises that. Add food and it drops. A pharmacokinetic review found that bioavailability falls from 79% when fasting to 64% when taken with food. That 15-percentage-point gap matters because the label says levothyroxine has a narrow therapeutic index: the line between too little and too much is thin.

For adults with primary hypothyroidism, the label has your doctor check TSH 6 to 8 weeks after any dose change, then every 6 to 12 months once the dose is stable. If your timing changes from day to day, so does absorption, and your dose may get adjusted for what is really a timing problem.

The Empty Stomach Rule

The American Thyroid Association (ATA) guidelines recommend taking levothyroxine either 60 minutes before breakfast or at bedtime, 3 or more hours after dinner. Both produce reliable absorption.

The 30-minute minimum comes from the FDA label. But 60 minutes is better if you can manage it. The ATA guidelines rank 60 minutes before breakfast ahead of 30 minutes for absorption.

Here’s what counts as "not empty stomach": food, coffee, milk, fiber supplements, and even some other medications.

The Coffee Problem

Coffee reduces levothyroxine absorption by 29-36%, according to a study in Thyroid. In lab tests, coffee bound some of the drug, though less strongly than fiber or antacids do.

If your morning routine is alarm → levothyroxine → coffee → breakfast, you need at least 60 minutes between the pill and the coffee. Not 15 minutes. Not "a quick cup while I get dressed." A full hour.

For a lot of people, that’s not realistic. If you can’t wait an hour for coffee, ask your doctor about bedtime dosing (covered below).

What Else Interferes With Absorption?

A 2021 systematic review in Pharmaceuticals compiled the evidence on substances that reduce levothyroxine absorption. Here’s what to watch for:

SubstanceAbsorption ImpactHow Long to Wait
Calcium supplements20-25% reductionAt least 4 hours apart (FDA label)
Iron supplementsSubstantial reductionAt least 4 hours apart (FDA label)
Coffee29-36% reduction60 minutes
Antacids (aluminum hydroxide)Significant reductionAt least 4 hours apart (FDA label)
PPIs (omeprazole, pantoprazole)May reduce absorptionTell your doctor so they can monitor (FDA label)
Soy productsVariable reduction60 minutes
Fiber supplements (psyllium)Small (89% vs. 80% absorbed, one study)60 minutes

The 4-hour wait for calcium and iron is the one that trips up people who manage multiple medications. The Synthroid label names "iron and calcium supplements and antacids" and tells patients "not to take SYNTHROID tablets within 4 hours of these agents." So if you take levothyroxine 30 to 60 minutes before breakfast, don’t take a calcium supplement with that breakfast. Move it to lunch or later, at least 4 hours after your dose. Your pharmacist can help you set the times.

The Wegovy pill also wants an empty stomach in the morning, and its label reports a 33% rise in levothyroxine exposure; see Wegovy pill and levothyroxine.

Levothyroxine Morning or Night: How to Choose

Morning dosing (the standard)

Morning is the standard schedule, and the FDA label specifically says "one-half to one hour before breakfast." The advantage is simplicity: wake up, take the pill, wait, then eat.

Morning works well if you:

  • Have a predictable morning routine
  • Can wait 30-60 minutes before eating and a full hour before coffee
  • Can take calcium or iron at least 4 hours after your dose

Bedtime dosing (the alternative)

A 2010 randomized crossover trial published in Archives of Internal Medicine found that bedtime dosing actually decreased TSH by 1.25 mIU/L compared to morning dosing. Free T4 and total T3 also improved. Why? Two possible reasons: slower gut movement at night, and no food or coffee competing 3+ hours after dinner.

That said, a 2022 pragmatic trial in 201 adults aged 60 or older found no significant difference between morning and bedtime. Both work.

Bedtime works well if you:

  • Can’t wait 60 minutes for coffee in the morning
  • Finish dinner at least 3 hours before bed
  • Don’t snack late at night

Practical Levothyroxine Timing Scenarios

"I already ate breakfast. Should I still take it?"

Take it, unless it’s almost time for your next dose. MedlinePlus says to skip the missed dose then, and never take a double dose. Absorption will be lower than usual, but levothyroxine’s long half-life, about 7 days, keeps levels fairly stable. Tomorrow, go back to your normal empty-stomach routine.

"I take blood pressure meds and metformin in the morning too"

Levothyroxine goes first. Take it when you wake up, then take your lisinopril and metformin with breakfast 30-60 minutes later. Most blood pressure and diabetes medications don’t interfere with levothyroxine absorption, but the food and coffee you take them with will. Ask your pharmacist first if one of them is colesevelam (Welchol) or oral semaglutide (Rybelsus or Ozempic tablets). Both are covered in the FAQ below.

"I take calcium and iron supplements"

Keep them at least 4 hours away from levothyroxine, before or after, as the Synthroid label directs. If you take levothyroxine at 6 AM before breakfast, calcium or iron can go at 10 AM or later, for example with lunch. Or take levothyroxine at bedtime and the supplements the next morning. Just don’t take them with a dinner 3 hours before bed, which is under 4 hours.

Building a Levothyroxine Routine That Sticks

Levothyroxine is one of the most prescribed medications in the US, with over 80 million prescriptions a year. You can’t just throw it in with your other pills at breakfast.

What helps:

  1. Set an early alarm. If you take it at 6 AM and eat breakfast at 7, the timing takes care of itself. If you’ve had the can’t-remember-if-I-took-it moment, that alarm needs to be persistent.
  2. Keep it on your nightstand. Whether morning or bedtime dosing, having the pill within reach when your alarm goes off removes the "I’ll take it in a minute" problem.
  3. Use a reminder that won’t let you forget. Regular phone alarms are easy to swipe away. Pillo uses persistent alarms that keep going until you respond, which matters when there’s a 60-minute countdown between your pill and your first cup of coffee.
  4. Track whether you’re actually consistent. If your TSH keeps fluctuating, timing inconsistency could be the culprit. Pillo’s adherence reporting tracks your medication-taking history, so missed days are easy to spot.

Frequently Asked Questions

Can I take levothyroxine at night instead of morning?

Yes, with your doctor’s OK. The ATA guidelines recognize bedtime as a valid option, as long as you haven’t eaten for at least 3 hours. A 2010 trial of 90 patients found bedtime dosing lowered TSH by 1.25 mIU/L on average compared with morning dosing.

How long after taking levothyroxine can I drink coffee?

Wait at least 60 minutes. A study in Thyroid found that coffee reduces levothyroxine absorption by 29-36%. If you can’t wait that long, ask your doctor about bedtime dosing.

What happens if I take levothyroxine with food?

The drug still works, but absorption drops. A pharmacokinetic review found bioavailability falls from 79% (fasting) to 64% (with food). If it becomes a habit, the label says your doctor should check whether your dose needs adjusting.

Can I take levothyroxine with other medications?

It depends on the medication. Calcium, iron, and antacids should be at least 4 hours apart from levothyroxine, which rules out taking them with breakfast after a morning dose. Most blood pressure and diabetes medications are not on the levothyroxine label's list of drugs that lower its absorption, so they can usually go with breakfast 30-60 minutes later. Two diabetes drugs are exceptions. The label says to take levothyroxine at least 4 hours before colesevelam (Welchol), and the label for Rybelsus and Ozempic tablets (oral semaglutide) reports higher levothyroxine levels when the two are taken together. Ask your pharmacist if you take either one. If you have diabetes, the label also warns that levothyroxine "may worsen glycemic control," so check your blood sugar as your doctor directs and report changes right away. If you miss a dose of levothyroxine, take it as soon as you remember, unless it’s almost time for the next one. Then MedlinePlus says to skip it and not take a double dose. For a broader overview of thyroid medications, see our guide on what to do if you miss a dose of thyroid medication.

How long does it take levothyroxine to work?

It may take several weeks to notice a change in symptoms, and the Synthroid label says the peak effect of a dose may not come for 4 to 6 weeks. For primary hypothyroidism, your doctor rechecks TSH 6 to 8 weeks after any dose change.

I forgot to take levothyroxine before breakfast. Can I take it after eating?

Yes, as long as it isn’t almost time for your next dose. MedlinePlus says to take a missed dose as soon as you remember, skip it if the next one is close, and never double up. Absorption will be lower with food in your stomach. Tomorrow, go back to your normal 30-60 minutes before food routine. If this happens often, ask your doctor about bedtime dosing.
